How to Register Your Child for Head Start/Early Head Start

To register your child(ren) for MYCAP Head Start/Early Head Start, you must first call for an appointment, then visit our main office or one of our three multiple sites in person:

IMPORTANT! You must bring the following information with you in order to complete the application process:

1. Medical or Insurance Card
2. Proof of income
3. Social Security cards for all family members
4. Birth certificate of child
5. Shot record
6. Custody or restraining order documentation (if applicable)
7. Picture ID

You will need to fill out several application forms in order to register. You may do this in person at the time of your appointment, or you can save time by filling out these forms in advance and bringing them with you along with the above materials.

After completing an application, you will be given a physical and dental form that must be completed by a physician and dentist prior to their child entering a classroom.

You will be notified whether or not your child has been accepted into the program. Children are selected by a committee made up of staff and parents, based on family needs and an approved criteria ranking point system.
